Collin Clark is a Clinical Assistant Professor in the Department of Pharmacy Practice at the University at Buffalo School of Pharmacy and Pharmaceutical Sciences (UB SPPS). He completed his Doctor of Pharmacy at the University at Buffalo. He then went on to complete a PGY-1 Pharmacy Practice Residency at the Veteran Affairs Western New York Healthcare System and a T32 Health Services Research Fellowship at the University at Buffalo Jacobs School of Medicine and Biomedical Sciences. His current clinical practice is in transitions of care at Buffalo General Medical Center. His research focusses on implementation and assessment of clinical pharmacy services to optimize medication use for older adults and improve health outcomes for patients during care transitions. He currently serves as the Secretary for the Western New York Society of Health-systems Pharmacists and as the Chair-elect of the Geriatrics Practice and Research Network of the American College of Clinical Pharmacy.
