Jennifer Lesher Walls, PharmD, BCPS is a Clinical Pharmacy Specialist at Penn Medicine Lancaster General Health. Dr. Walls is a graduate of Duquesne University and completed a post-graduate residency at the Penn State Milton S. Hershey Medical Center. She has been practicing in both hospital and ambulatory care environments for over 25 years with specialization in the areas of anticoagulation, diabetes, and transitions of care. Dr. Walls has been a pharmacy preceptor for Lancaster General Health's post graduate residency program since its inception in 2010 offering rotations in internal medicine, clinical monitoring, and transitions of care. She has been awarded preceptor of the year in 2014 and 2020, participating regularly as a resident research preceptor. Dr. Walls was involved in establishing the first ambulatory collaborative practice agreement at Lancaster General Health in warfarin management and continues to practice anticoagulation management at the Advanced Heart Failure/Left Ventricular Assist Device Clinic. Dr. Walls has been project lead for Penn Medicine Lancaster General Health in researching, developing, and implementing their pharmacist discharge medication reconciliation program that has been in existence since 2017 and was most recently published in the American Journal of Health-System Pharmacists for her research on prevented harm and cost avoidance associate with discharge medication reconciliation while utilizing a triaging tool for pharmacists in selecting highest risk patients.
