Corey Dinunno is an acute care clinical pharmacy specialist with a practice focused on optimizing complex anticoagulation management. He was lead author on the publication titled, “Direct oral to parenteral anticoagulant transitions: Role of factor Xa inhibitor-specific anti-Xa concentrations'', which found that using FXaI anti-Xa concentrations to guide oral to parenteral anticoagulant transitions was associated with a reduction in major bleeding events. He has also contributed to several other manuscripts currently in peer-review evaluating the role of FXaI-specific anti-Xa concentrations across different spectrums of care. Corey has contributed to his institution’s System Anticoagulation Committee and has authored several institution-specific protocols aimed at maximizing the safety and efficacy of anticoagulant transitions, such as a perioperative anticoagulation guide. His clinical practice and research work has led to the development of an oral to parenteral anticoagulation guidance document which incorporates FXaI-specific anti-Xa concentrations to help guide in the decision-making process. This guidance document is currently being utilized at one of the largest institutions within the Texas Medical Center. In his day-to-day practice, he frequently consults for an interdisciplinary team tasked with evaluating and managing patients with clinically complex anticoagulation plans (including but not limited to those on mechanical circulatory support, patients with acute coronary syndrome on prior DOAC therapy, patients requiring anticoagulant reversal, and patients transitioning from oral to parenteral therapy with heightened bleeding/clotting risks).
