Adley Lemke, PharmD, BCPS, BCTXP graduated from the University of Minnesota College of Pharmacy and went on to complete his PGY-1 and PGY-2 in Solid Organ Transplant at Hennepin Health, Minneapolis, MN. Following post graduate training he joined Mayo Clinic, Rochester, MN where he cares for heart, kidney, liver, lung and pancreas transplant recipients in hospital and clinic settings. He enjoys teaching and precepting, and serves as the assistant residency program director for the PGY-2 Solid Organ Transplant program. He is recognized for numerous quality improvement initiatives at Mayo Clinic, and is passionate about improving transitions of care and medication access. Nationally he has served on the American Society of Transplant Pharmacist Community of Practice education and policy workgroups. His research interests include infection prevention, diabetes management in transplant recipients, pharmacogenomics, and initiatives advancing equity, inclusivity, and diversity.
