Michael Duffy, PharmD, RPh, BCACP, is an Operations Coordinator at University Hospitals (UH) in Cleveland, Ohio. Dr Duffy's current team consists of 40 primary care ambulatory pharmacists and oversees all primary care operations that serves over 500,000 patients in Northeast Ohio. Michael earned his Doctorate of Pharmacy from Northeast Ohio Medical University (NEOMED) in Rootstown, Ohio. After graduation, he completed his Postgraduate Year 1 (PGY1) Community Residency at The Centers for Families and Children in Cleveland, Ohio. At University Hospitals, Michael is passionate about advancing the profession of ambulatory pharmacy in all outpatient settings. Michael pushes his team to practice at the top of each member's license. Through the use of broad collaborative practice agreements, Michael's team is focused heavily on impacting patient outcomes for a variety of chronic disease states. Michael's most impactful achievement at UH is the development and implementation of a post-discharge clinical pharmacy service aimed at reducing 30 day readmission rates. This service has resulted in a statistically significant difference in readmission rates for the system. Michael is looking forward to continuing to grow his ambulatory pharmacy team, as well as continuing to expand and launch initiatives to further impact patient care.
